Evaluation

Your one hour evaluation will consist of an in depth interview, including obtaining a detailed history and utilizing multiple screening tools. The screening questionnaires are used to assist with diagnosing, and as a continued measure of progress throughout treatment. We will discuss individualized treatment options, goals, and plan our follow up. 

You will leave our meeting with a sense of hope, and the knowledge that I care and am on your side, committed to helping you achieve your goals of a healthier life. Please note that any telehealth services require the patient to be physically located in the state of Florida during their appointment.
